Where did everyone else go?

If World of Warcraft has 11.5 million players and its latest expansion The Wrath of the Lich King has sold four million copies, what are the other 60+% of the players waiting for? Yes, it is huge news from Blizzard that they have sold 4 million copies of Lich King, but it really makes me wonder. The press release is a bit vague on whether or not the copies sold are boxed copies or digital copies. If the 4 million is only the box tally, how many have upgraded online? If 4 million is the combined total, then what is the breakdown for how many were boxes versus downloads? I am curious how many people upgraded digitally, but of course we will probably never know since the shroud of online sales secrecy is so thick.
