Phase 28 Questionnaire: Multiple-choice questions on character creation choices, game-play history, and play style. Should take about 5-10 minutes to fill out.

Stories - Role-Reversal & Therapy: Two unrelated open-ended questions in this survey. One is about people you play with where the roles get reversed from what they are in the physical world. The second question is about whether playing an MMO can be therapeutic or whether it worsens our problems if we escape into them.
